quick question - is the starting position RNG?

Yes

There are 3 decks of cards with the gems at the 3 levels -- four of those are dealt out for each row.

Similarly the four nobles are picked randomly from the pool of nobles.

In a 3-player game. There is one more noble available than there are players.
