What is up with players refusing to start the game?

Why do some players join a table but then refuse to start?

This has happened a few times lately and I can't imagine any other reason but strong players not wanting to lose rank to an unknow novice player.

Are there other reasons I'm not aware?

BTW, I didn't find a topic with this specific question.

How can I end a game where the adversary doesn't move?

As in the title, one player is not moving. It's his first move

How can I end the match without selecting "lose the game" or incurring in a penality of leaving the game?

This never happened so I don't know what to do. I'm a relative newbie in bga.

I interpret it just as a polite handshake, as in chess. Since I can't physically do that, gg stands as widely known and understood symbol. Likewise, if an opponent actively refused (not just disinterestedly missed it) to handshake, or to voice it, I would interpret that they were pissed by the game ...
