I agree that "should only be [made] to fix clear and obvious mistakes" is bad guideline if taken literally. I do feel it should be more like "don't make changes just for the sake of making changes", in other words you want the change to happen for a reason.
I've done full passes on some translations where I've redone 60-80% of them, not because they were all wrong, but mostly because they just were made by different people and lacked consistency, had very different translations for some words and like. You want the translation to work as whole, not just for single sentence.
