It is rare that I think winning by booting someone is more enjoyable than actually playing the game I have come online to play.

Personally, in the original scenario where my opponent has been consistently playing a game for that length of time, I would usually choose 4 and 5.

- Communicate first. Hopefully receive a response and the turn will be taken.

- If there is no response, I will assume internet issues or called away due to an emergency. It's likely (although not a given) that I'd switch the game to turn-based. I can allow some grace, but also I don't have to wait around if I don't have the time to.

It's rare that I think keeping myself stuck at a table with someone playing slowly is more enjoyable than booting them and trying to find a new opponent that can play at the speed the table is set for.

I consider the time bank to be sufficient "emergency time" and once it's used up I have no problem booting the player and looking for a new table.

Of course I play almost exclusively turn-based so that time bank is usually 24 hours. I have had too many tables of "1 move/day" where one of the players is consistently taking 30-40 hours between turns (which means it can take a week for their time bank to finally be emptied to the point that I can boot them) for me to have an patience with people who run out of time.

There has been the rare occasion where my opponent has been playing relatively quickly for most of the game and then suddenly starts running out of time, in those instance I will give some extra leeway before booting. However, most of the time I can usually tell within the first couple turns that this player isn't willing to keep up with the speed of the table and I'm going to be booting them before the game ends.
