Fails to Play? what is it?

Sometimes I will see a Fails to Play message. But, i cannot find the rules that cause it.
One example: I clue red on three cards. Assuming player will discard the oldest red. When he discards, we get a fuse/penalty! Is it because a discarded card was identified (as red)?

That means he didn't discard it but wanted to play that card. But it wasn't playable so he failed at it

Right, i think he played the left red instead of discarding the right most red. (Which i now understand is the convention for a Color clue.)
Fails to Play (correct sequence) is what is meant.

"${player_name} fails to play ${value_displayed} ${color_displayed}" has been changed to "${player_name} plays ${value_displayed} ${color_displayed} and fails" for clarity.
