So it says to become a reviewer, "Your reputation on BGA must be absolutely perfect." Will you be blocked from being a reviewer for the rest of time if you take any damage to your reputation? Or can you recover your reputation and become a reviewer after having a bad reptuation?

No, it comes back quickly enough. I'm not even sure you're required to have 0 penalties on your profile, but you definitely need 100% reputation at the moment you are approved to be a reviewer.
