Three Cataan rules are wrong on Cities and Knights.
1) Knight protection against barbarians, in no rules version should a player with one active knight and one city lose their city when barbarians attack. Fix the algorithm.
2) a 2 & 12 should never be side by side on hexes.
3) city walls should protect cities from robber placement as well as allow extra card holding.
Fix these items.

You have some wrong rule interpretations:
1) There are many situations where you can lose your only city despite having one active knight.
Situation I) Player A has one city and one active knight, Player B has two cities and one active knight. Player C has no cities and no active knights. Player A and B will lose a city each.
Situation II) Player A has one city and one active knight, Player B has one city that has already been upgraded to a metropolis and no active knights. Player C has no cities and no active knights. Player A will lose his city.
2) It depends on the game options. BGA provides 4 different setup rules for "Terrain tile layout". If you choose "Variable setup with random numbers" 2&12 can be on neighboring tiles as far as I remember (as can 5 and 5,...). Choose "Variable setup" as an option instead.
3) City walls do not protect cities from robber placements according to CATAN rules.

There are 2 possible outcomes of this battle:
• If the barbarians are stronger, they are victorious! If the
barbarians win the battle, they pillage one or more cities.
Pillaged cities are reduced to settlements (replace the city
piece with a settlement). The barbarians pillage one city
belonging to the player who had the lowest total strength of
active knights when the barbarians attacked. Any player who
contributed no knights are automatically considered to have
the lowest total strength of active knights. If various players
tie for having the lowest total strength of active knights,
each of those players loses 1 of their own cities.
Important: When determining which player has contributed
the weakest group of knights, do not count any player who
has no cities or any player who has only metropolises. These
players are immune to the effects of the barbarian attack.
If the barbarians pillage a city that has a city wall, the city
wall is destroyed and removed from the board.
These are the rules available from the official Catan site.
Players with only one city and an active knight can lose that city as long as all other players with cities have more (or all are equal lowest) Knights.
