Can someone explain how the rules for who goes first work? It feels like, at least in some games, that one player always or nearly always gets to go first, giving them a major advantage.

Whoever takes the left most tile this round goes first next round. Yes you can hog the first player token if you only take the left most option every time but it doesn't give you a lot of options for scoring points.
