Just a clarification if it's a rule or a bug?

In a recent run, I was playing as the quartered. I had the red scrolls upgraded twice, so says that any effect applied to the card in front will also affect the card behind, but I played a poison taken on one of the 1st cards and it didn't transfer to the card behind. So just clarifying that when it says effect does it just mean damage, because at the red scrolls say specifically damage, not effects. So if anyone can clarify that, let me know. And if it's a bug so that I can report it.
