The way I understand the rules, if a player is dealt a creeper at the start of the game, the creeper goes in front of that player, and they draw a replacement card, the same as if that player had drawn the creeper on any other turn. The only issue with the number of players plus one setup rule is that in the event that both players chose the same setup not to play, then there’s no good way to decide which of the two leftover setups to use. This is an interesting topic. From my experience, the random map set-up puts very little luck into the game, and even then only if both players are at a very high level. I think that the purpose of putting the 300+ ELO minimum on the Top ELO Achievements was to make it so that the awards would not go to all of the players who played a game just after it was created, but it does nothing to prevent the achievements from going to players who got lucky in their first 10...
