Why do you get hurt when you get hit by Duel and don't Bang?

The rulebook states that Banging is not mandatory so if you're afraid you can just refuse the Duel.

Also how come the Saloon only heals 1 Life to the user instead of 2 ?

You cannot "refuse" a duel. Banging is not mandatory, but if u dont, u get hit.

because it heals 1 to you and 1 to every other player, not 1 to you and 1 to every player. The symbols are pretty clear, if it was like you said, gatling would hit you too for example
