Why does arena mode NOT auto start?

This is only a guess:

If you are trying to join a table in simple mode but none are available, the site will tell you if there are people waiting in arena. Perhaps it is to allow people who wanted to play a simple game a chance to check out the arena game without committing?

maybe but it seems unfair, especially when youy wait for arena game a long time... go do something, and then realize game is starting and youve been kicked...

I can confirm it is unfair, and it never seems to wait for me. It just launches without me knowing and I get expelled. So I don't know how you're seeing different. Arena auto start is such a pain. I wish they'd fix it

My guess is that player is taking a move on a turn based game, so he can't see the start screen and can not auto-start as usual.
