Conspiracy changes Medieval Empire to Theocracy?

What would cause a conspiracy to change an empire to a Theocracy (religious version of the empire map "card")? I had this happen in my games, but I don't understand what triggers this. I didn't see anything in the rules about this.

Are you sure it was a conspiracy and not a religious war?

Would be great if you could provide a link to a game where it happened, move number and some information on what card was played.
