Why realtime only?

I see no reason the game wouldn't work well turn-based. Why is it realtime only?

I see a very good reason. You can play a "mirror" card as a reaction at any time, out of your turn. In turn-based you'd have to be constantly online and monitoring the table to do that.

I understand some games are best played in real-time mode, but banning turn-based is not always a good idea.
In some games, card plays can be queued in advance to make turn-based games viable.
And I think this is a possibility in Ekko as well.

Yeah some of the decisions the dev team have taken on this are a little disappointing in my opinion. It would work fine implemented in a similar way to Catan trading, with game options to either alert every time a mirror is available, and preferences to only alert when it is your turn, or alert every time a card is played so one can manually check for a mirror.

I think as this is a small game they have cut corners to keep it simple as they don't want to go into too much depth in the implementation.
