It is easier to explain why 'which card do you want to keep' would have been a better choice if you consider a situation with 3 or more cards. It is human nature to focus on the card you most want,not the card you least want.

This inconsistency is a real problem and should be fixed. BGA's user interface is already problematic without simple things like this also being a problem.

Ease of use and consistency are important.

This has already been discussed several times, at least in the bug report section. There is a consistency : the game always ask you the minimal number of clicks needed and will always display a big trash icon on the selected card(s) if the card(s) is going to be discarded.
I am still puzzled to learn that people keep misclicking despite that, really. Do you have any specific examples that you find counter-intuitive ?

if you get adapt 3 you go up to 4-5 end goals typically and have to discard 3 instead of keeping 1-2 to keep.

I would choose to make the interface with Keep always when you have to keep 1-2 cards out of a bunch (adapt, scavenge, hunt, perception, resistance, assertion) and Discard only when you're purely discarding cards (digging, pouching, sunbathing).

Adapt is a little counterintuitive and more clicks now.

Also Discard icon could definitely be more clear.
You are right about Adapt. Both situations where you need to discard more and to keep more may happen. Maybe I will change this in the future then.
I will also make the trash icon highlight more visible, and maybe add icons to the confirm button to make it more distinguishable if you are confirming to discard (trash icon) or to keep (cards/hand icon). Maybe I will add the icon to the title bar as well.

If I had to design a UI from scratch, I'd probably have two big containers, one for cards to keep, one for cards to discard. Initially all cards would be in one of the containers (or none), then you'd just click on a card to move it to the other container. The confirm button would be greyed-out until you have the correct number of cards in the appropriate container. Just an idea in case it helps.
