Homunculus Rune

This rune says that, when undamaged, attacks gain the ability to target the third enemy slot. What qualifies as an attack? The Poisoned round 0 ability to deal damage was not able to target the Timekeeper in slot 3, nor did an item (meat skewer) gain the ability to target the Timekeeper back there. Shortly thereafter we both took damage so I was unable to determine what, if anything, actually benefits from the rune.
