From your question, I'm unsure if you were moderated twice for a single action in a game (e.g., two different opponents reported your behavior from a single game), or if you displayed the same negative behavior twice in two different games. I will try to address both.
The moderation policy

In my mind, it is reasonable to conclude you should only be penalized once for one action, regardless of how many people were offended and reported the issue. If that's the case, it may be worth reporting to the moderators and letting them know you were penalized twice for the same action.
However, given that the moderation penalties were applied days apart, it is more likely that you displayed the same offensive behavior twice in two different games. If that is the case, then yes you can be penalized more than once. The moderation policy specifically mentions repeat offenders.
