I don't think it's an optical illusion. A second thumb was clearly intentionally created. This shape didn't just happen by accident.

That's something we definitely agree on. There's a piece of background colored brown instead of red between hand and neck, which could be mistaken as a finger at first sight, but if you stop to count, there's only 5... unless people are referring to her left hand.
That's her hair a little in the shadow.
