I don't see any reason why not just half their ELO points
A harsher karma penalty can be argued for, but halfing elo points is simply ridiculous. Quitting or exceeding the time limit are bad practices, but there is no reason someone better at the game should be punished more.

This is possible with a premium account. If you don't have a premium account though, you won't be able to play with someone from the same IP address.
The player from the different location will see a same IP address warning but since you're not strangers on the internet that won't be a problem.

If a player declined to abandon the game collectively, the proposer won't be able to make the same proposal for a certain period. I'm not exactly sure how long that period is, but it definitely lasts more than 2 minutes. It's technically impossible to spam the abandon proposal.
